cARE cOORDINATOR VACANCY

  

This is an exciting opportunity for a highly skilled and motivated Care Coordinator with a strong care background and a passion for high quality care to join our team.


You will be responsible for the growth and development of our domiciliary care and outreach service ensuring the delivery of outstanding quality care. 


Job Type: Permanent/ Full Time 40 hours a week Monday to Sunday (Days and times to be confirmed)


Salary: £25,974 Per Annum

Location: Pudsey, Leeds


In this varied role, you will be involved in:

· Liaising with Social Workers and other professionals as necessary and ensuring that client files are kept up to date at all times

· Liaising with clients and their families to understand their care requirements and developing person centred care plans and risk assessments prior to the commencement of the service or within 48 hours in emergency circumstances.

· Ensure care plans are reviewed monthly or when a change occurs

· Covering support packages on a weekly basis or as and when required to meet the needs of the service

· Ensure that staff and clients are matched to meet client needs

· All complaints/incidents and accidents to be investigated and be reported immediately to the manager and recorded on the correct paperwork and  system

· Ensuring compliance with all relevant regulations and standards, maintain our commitment to excellence

· Being part of the Out of Hours rota and responding to emergency calls from staff, service users and families

 

· Ensure that all tasks are  completed within TLS social Care’s policies and procedures, CQC fundamental standards and all contractual and legislative requirements. 

· Maintain appropriate records to meet the needs of the service, observing and promptly reporting back any changes or concerns in your service user’s circumstances or condition. 

· Contribute to and work effectively as part of a team. 

· Deliver support in a manner which is sensitive to the service user’s culture, disability, race, age, sexuality, marital or civil partnership status or religious believes. 

· Engage and work closely with unpaid carers such as family or friends for the benefit of the service user. 

· Promote anti-discriminatory practice 

· Protect the confidentiality of service users and of TLS Social Care. 

· Promote the dignity and privacy of service users. 

· Act promptly and appropriately to protect service users from neglect or abuse 

· Participate in staff development (including supervision), training and performance appraisals as required. 

· At all times, present a professional and caring image of TLS Social Care and demonstrate behaviours reflective of TLS Social Care values. 

· Any other duties as delegated by line manager. 


About you:

· Excellent communication skills over the phone, face to face and in written communications

· Strong ability to develop and maintain relationships

· A clear understanding of issues surrounding confidentiality

· Be confident, reliable and dependable.

· Possess excellent organisational skills.

· Excellent customer service skills

· Ability to support back up "On Call" duty rota

· Proficient in the use of databases, Outlook, Word and Excel

· Ability to prioritise

· Good attention to detail

· Respect and appreciation of quality standards required in the Care industry

Qualifications & Experience:

The candidate will either hold, or be working towards, an NVQ level 3 qualification in Health & Social Care.


Due to the need to support the on-call function, which may require you to support team members out in the field in emergency situations, and also the requirement to travel throughout the community to conduct new client assessments, you should have a driving licence and access to a vehicle.
