Who we work with
We offer outreach support to those living with Learning Disabilities, Autism and Mental Health issues. We pride ourselves in providing support which enriches our service users' wellbeing by enabling them to build their confidence and independence in all aspects of life.
Community Outreach Services
Our Outreach service is aimed at providing support within the home as well as in the community to enable you to live a life of your choice with the right care or support. We offer the following support:
- Maintaining the upkeep of your home including support with correspondence
- Managing money, paying bills and working to a budget
- Support to access leisure activities in the community
- Understanding your rights and responsibilities of a tenancy agreement
- Support to attend social activities and events
- Support to attend health appointments, education and seeking employment.
- Assisting or prompting medication
- Sitting service or providing respite for informal carers and families
- Support with house hold tasks
- Meal planning and going food shopping
