The initial stage is for you to contact us 01274 008848 or complete our Service Referral Form, we will then arrange an assessment.
For individuals who are assessed as lacking capacity to decide about their care and treatment, it is important that they are supported to participate in the decision-making process and we do this by involving carers, representatives and professionals involved in their care.
We will come out to meet you and to discuss about the care and support you are looking for or need.
Following this, we complete a support plan with you to finalise the type of support you want or need.
During this time we also discuss about your staff preferences (male or female), days and times of support
We then confirm if we are able to meet your care or support needs and proceed to the funding stage which normally consists of how you will be funding for your care and support.
A self-funder is typically an individual who organises or funds for their own care or support as opposed to receiving financial assistance from the local authority or NHS.
You may be eligible for the local council to pay towards the cost of your social care if your savings are below £23,250 and from October 2025 this will rise to £100,000 in savings. The council will determine how much you have to contribute towards the cost of your care by completing a financial assessment.
Following a care and support plan completed by a social worker, you will get a Personal budget. You can choose to get your personal budget in three ways or a combination of all three:
A personal health budget is NHS money used to pay for a person's health and wellbeing needs. You decide what you want your support to look like with the agreement of your NHS team. This gives you choice and control about how your needs are met and gives you flexibility about what your support looks like.
Personal Health Budgets are for people with long-term complex health and care needs funded by the NHS and known as NHS Continuing Healthcare.
Some adults can get Section 117 Aftercare funding for their mental health needs.
